While a small percentage of men experience it, researchers have largely agreed since 1978 that cellulite is a sex-related condition dominant in post-pubertal women.[2] Though researchers don’t know the exact causes of cellulite, many speculate that estrogen plays a significant role in its appearance since many women see an increase in cellulite during puberty, pregnancy, and menopause.[1] Cellulite is commonly found on the thighs, upper knees, buttocks, and sometimes the abdomen. While diet and exercise assist in losing fat around other areas of your body, cellulite is different – it doesn’t respond to diet and exercise. That’s because cellulite is a structural problem in subcutaneous tissue rather than an overabundance of fat cells. Under the skin, women’s fibrous septa – the thick bands that connect the skin to the muscle – are vertical as where men’s are horizontal. With its vertical orientation, the fibrous septa compress the underlying fat between the skin and muscle, causing the fat to create an “orange-peel” appearance under the skin.[3]
Qwo is made up of collagenases clostridium histolyticum-aaes (CCH), a naturally occurring enzyme that, when injected into localized cellulite, releases the tension of the thick fibrous septa bands. Over time, the enzyme breaks down the fibers tethering the skin to the muscle, allowing the underlying fat to redistribute evenly under the skin. As this process occurs, the sight of cellulite diminishes and the skin around the buttocks appears smooth and balanced. A 2021 study following 843 Qwo patients found that this simple injection significantly improved cellulite with little to no adverse side effects.[4]

First, there is incipient cellulite: cellulite that is hardly noticeable to the naked eye and only becomes apparent when the area is pinched.
